  Updated install so migrate and upgrade will complete successfully

Had to take out updating of search cache for php and template pages as errors could happen depending on what the page was trying to do.

Duplicate Usernames fix refactored to take into account case and accent insensitive mysql server setups.

During migrate update of xmlsitemap caused issue since $_XMLSMAP_CONF is not loaded correctly. Setup  XMLSMAP_update to be bypassed if this happens.
