  Added ability for plugin to automatically add user profile fields to User Profile and Last 10 Block

Feature request #471.

- Under My Account these fields will display on the About You tab
- Display on User Profile page including items in Last 10 Block
- Fields are editable under Admin User Editor as well

Manual way requiring of updating templates removed and replaced with
automated way.
