Comprehensive CovidWatcher survey for first-time participants. Come back and answer surveys below often. After you complete the survey, be sure to explore the data.
Introduction: We are asking you to take part in a research study to better understand the development of the coronavirus COVID-19 pandemic and to identify potential hot spots through a mobile app and anonymous surveys. These valuable data can help prepare cities, public health officials, and healthcare institutions in their response to COVID-19. This study is a collaboration between the Department of Biomedical Informatics and the Division of Infectious Diseases at Columbia University and the Urban and Social Policy Program at the School of International and Public Affairs. Individuals in the New York City area and throughout the United States can complete a set of surveys that ask about their health and symptoms, their access to medical care, their employment status, their mental and emotional health, and several others. The participants can choose to take one or several surveys, and to take them only once or as often as they would like. For most people, participating in these studies will take no more than a few minutes every day. 

Risks: There are no risks associated with participating in this study.

Benefits: You may or may not personally benefit from this study. Your participation may help us gain valuable data to learn more about the coronavirus pandemic.
Permission for future contact: You will be asked for your permission to contact you for future phases of this research and for future research studies. If you choose “Yes”, a member of the research team may reach out to you over your preferred way of being contacted (phone or email). For each future study you will only be contacted once. If you choose “No”, you will not be contacted and your involvement in the current study will not be impacted. 

Confidentiality

No personal or identifiable information will be collected in the anonymous survey. The only information being collected are your survey responses.
At the end of the survey, you will be assigned a PIN number. Each time you take another survey, you can enter this PIN number to connect your responses, and you will remain anonymous.

In addition, at the end of the survey, you will have an option to enter your phone number and/or email address, if you wish to receive daily reminders about the surveys. If you choose to enter this information, it will be stored on a secure server at Columbia University Department of Biomedical Informatics separately from your survey answers. Only study investigators will have access to your phone number and/or email if you choose to share them.

Finally, when you access the survey, you will have an option to enter your email address to be added to CovidWatcher mailing list. If you choose to enter your address, you will receive periodic emails with updates about the project. 
The following individuals and/or agencies will be able to look at, copy, use, and share your research information:   
   - The investigator, Columbia University Medical Center study staff, and other medical professionals who may be evaluating the study
   - Authorities from Columbia University, including the Institutional Review Board ('IRB')
   - The Federal Office of Human Research Protections ('OHRP')

 

Whom do I call if I have questions or problems? If you have any questions or concerns about this study, use the anonymous question form on our website and our research team will answer your questions on our FAQ page. Additionally, you may contact the research team with any questions, feedback, or concerns about the study at covidwatcher at columbia.edu.

Taking part in this study is your choice. You can decide not to take part in or stop being in the study at any time for any reason. If you no longer want to participate in this research, do not participate or continue completing any further surveys.

Also, even if you revoke (take back) this consent and authorization, the researchers and the sponsor (if applicable) may continue to use and disclose the information they have already collected.
